Eldersource is the single source of information for all disability and aging related issues
Eldersource is a legal joint venture of Catholic Family Center and LIFESPAN. This program has served Monroe County residents since 1995 by providing a single access point for obtaining information, guidance and care management services for older adults and their caregivers. Eldersource is staffed by social workers with expertise in the field of geriatrics.
Mission: In 1999 Eldersource was designated by Monroe County and the United Way as the single information and referral number for older adult services.Products:Eldersource services include:

Geriatric Screening Assessment:
•Comprehensive in-home evaluation by a professional geriatric social worker
•Written plan of care for immediate and long-term needs
Care Management Services:
•Assistance with implementation of care plan
•Recommendation of community options
•Referral assistance and ongoing coordination of services
•Family counseling and caregiver support
•Periodic reassessment and updating of care plan
Caregiver Consultation:
•Identification of caregiver needs
•Supportive counseling as needed
•Development/coordination of care plan
Phone Consultation:
•Assessment of client status
•Provide list of appropriate community housing options
•Advocate for family/client with facility
•Guidance with paperwork completion
